GOAL
Make Carroll County a place where people can age well in the communities where they live.
OBJECTIVES
- Increase resources and educational opportunities to make Carroll County communities dementia friendly.
- Increase coordination of services for older adults with chronic conditions.
- Work with Carroll County communities to make them more liveable for older adults and all people.
- Enhance the coordination and increase the utilization of Advance Care Directives.
MEETING DATES
The Aging with Connection and Purpose Workgroup meets quarterly on the 3rd Wednesday of March, June, September, and December.
