Aging with Connection and Purpose

GOALS

Improve the health of older citizens of our communities by enhancing connection and purpose through collaboration with community partners address multiple social determinates of health, including nutrition, transportation, housing, home healthcare, and community engagement.

MEETING DATES

The Aging with Connection and Purpose Workgroup meets quarterly on the 3rd Wednesday of March, June, September, and December from 9:00-11:00am in the Sununu Room at the Tri-County Community Action Program Resource Center in Tamworth.

Next Meeting:  Wed. Dec. 13th, 9:00 am - 11:00 am
